What is Choose Well?

At St. Joseph and Covenant Health, our vision of bringing people together to provide compassionate care, promote health improvement, and create healthiest communities starts with each one of us. To achieve this, we use Choose Well, our employee wellbeing benefit program, as the key to unlocking our well-being potential.

Why is the program different this year?

We’re immensely proud of the wellness outcomes we’ve achieved together with Choose Well over the past few years, and we’re excited to announce new enhancements to the program to keep you and your colleagues engaged and earning rewards for your well-being achievements all year round.

Who is eligible?

We value every employee’s well-being-all employees are now eligible to participate.

Is participation mandatory?

Enrolling in Choose Well is voluntary, at no cost to you.

Is my privacy protected?

Choose Well is administered through partnerships with Virgin Pulse and Quest Diagnostics who protect your information in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Therefore, your personal health information will remain confidential and only be used as permitted by law. It will never be shared with your employer and will never be sold. Virgin Pulse and Quest Diagnostics are not telemarketing services.

Points

How do I earn points?

You earn points for tracking your completed daily activity and steps, tracking Healthy Habits, and participating in challenges and competitions. The more daily activity in your routine and the more you participate in the program, the more points you can earn. These points will continue to add up and push you through the levels in your program, so you can earn more rewards! Check out the “ How to Earn” page on your Virgin Pulse member website for a list of all of the earning opportunities that are available to you!

What are big point opportunities?

  • 1,000 Health Assessment completion
  • 14,000 Primary Care Visit form submission
  • 2,000 Just Be Mindfulness Journey: Coming Summer 2017
  • 2,000 Your Pilgrimage Journey: Coming Fall 2017

How many points do I have to earn?

Your goal is to get to through all 4 levels by December 31, 2017. This will ensure that you receive the full amount of benefits next year.

What is the difference between points and PulseCash?

As you play the game you will earn points. As your points add up you will hit levels that are associated with a PulseCash reward amount that can be instantly redeemed.

  • Level 1=8,000 points
  • Level 2=20,000 points
  • Level 3=32,000 points
  • Level 4=44,000 points

Pulse Cash (rewards)

What is PulseCash?

Your points that you earn throughout the levels are turned into money. Your money earned is called PulseCash.

How much can I earn?

You can earn up to $850 dollars* in PulseCash.

* Employees covered under a collective bargaining agreement receive benefits in accordance with the terms of their contract.

How do I spend my PulseCash?

There are several options for redeeming your cash. It’s your choice!

  • Direct Deposit (deposit your PulseCash into a personal checking or savings account)
  • Gift Cards to iTunes, Amazon.com, or Target (physical gift card that will be mailed to you)
  • PulseCash Card (acts like a debit card or cash card; can be used anywhere Visa is accepted)
  • Upgrade your device, or get some new accessories for your pedometer.
  • Donations can be made to serveral oraganizations.

How do I redeem my PulseCash?

Website: If you wish to use direct deposit or get gift cards you will need to use the website.

App: If you redeem through the App, your only options at this time are to purchase trackers or exercise equipment. Go to the menu and press “Virgin Pulse Store”.

Are Virgin Pulse rewards considered taxable income?

Redeemed points, or Pulse Cash, are considered taxable income and will be reported on individual paychecks typically within one month of the date of earnings. * Employees covered under a collective bargaining agreement receive benefits in accordance with the terms of their contract.

Who do I contact if I was taxed for the incorrect amount of PulseCash rewards on my paycheck?

First, contact Virgin Pulse at (888) 671-9395 to confirm your PulseCash earnings are correct.

Then, contact payroll by e-mailing AskHR@stjoe.org to report the error.

Who should I contact if I have questions about my Virgin PulseCash earnings?

Employees should call Virgin Pulse at (888) 671-9395 for any account inquiries.

Can I earn points while I am out on leave?

Yes, you will continue to earn points while you are out on leave. Points can be redeemed at any time through the rewards center in your Choose Well account powered by Virgin Pulse.

Does my PusleCash expire?

No, your earnings never expire. They will stay in your account until you redeem them.

Primary Care Physician (PCP) Visit Form

Why is it important?

One of the most important things that you can do for your well-being is to visit your primary care provider (PCP) annually to set and discuss health and wellness goals and issues.

Will I have to pay a copay for my visit?

Your copay will be determined based on your medical plan. Call the Benefit Service Center at (800) 306-4363 for more details.

Where can I access the form?

There are two ways you can download the PCP visit form:

  1. Go to www.sjhschooswell.org, click on PCP form at the top menu.
  2. Through your Choose Well account Go to Programs section and click on Primary Care Visit Form.

How long does it take to process?

Allow seven days for processing.

Can I go to an urgent care or employee health to complete?

The purpose of the PCP Visit Form is to encourage employees to engage with their primary care providers in at an annual physical.

The physician extenders working in the employee health clinic are not intended to replace the relationship that patients should develop with their primary care provider and the network of care programs that those providers are trained to utilize for care management referrals.

Heath Assessment

What is it?

The Health Assessment is a health questionnaire through our partner, Virgin Pulse, and is used to provide individuals with an evaluation of their health risks and quality of life. It takes about 10 to 15 minutes to complete the questionnaire.

How many points do I earn for completing the Health Assessment?

You will earn 1,000 points by completing.

When do I have to have it completed by?

You must complete between May 1, 2017 to Decmber 31, 2017.

Where do I access it?

  1. Log into your Choose Well account.
  2. Click on “Programs” in the top-navigation menu. If a drop-down menu appears, click on "Company Programs".
  3. You'll be redirected to the "Programs" page, which contains information about the additional wellness programs and resources brought to you by Virgin Pulse and your sponsor.
  4. Locate the "Health Assessment" section on the page.
  5. Click the link within "Click here to start your assessment!" to begin your Health Assessment.

Where can I view my Health Assessment?

Once you've completed the HA, you'll be redirected to a report of your results. If you'd like to access this report, complete the following steps:

  1. Log into your account on the Virgin Pulse member website.
  2. Click on “Programs” in the top-navigation menu. If a drop-down menu appears, click on "Company Programs".
  3. Locate the "Health Assessment" section on the page.
  4. Click the link within "After you take the assessment, you can always return to your results here.", and you will be redirected to the report of your Health Assessment results.

Virgin Pulse

What is Virgin Pulse?

Virgin Pulse is our partner who powers our new and improved Choose Well platform. Our tools, powered by Virgin Pulse, can help you build good lifestyle habits across all domains of well-being; nutrition, sleep, mental focus, physical activity and much more. Your program will be personalized just for you. Stay on your path with positive feedback, education, friendly competition and rewards to create an irresistible experience. The best part is it’s easy to use.

Why have we partnered with Virgin Pulse?

Family and Friends

Can my friends and family members participate?

You can invite up to ten of your outside friends and family to join you in the program in addition to your co-workers friends. Friends must be fourteen years of age or older. Those ten friends will only be able to see you and the others you invited from outside of your company, not your work friends.

Can friends and family earn PulseCash?

No, they do not earn PulseCash.

How do I add family and friends?

Website:

  1. Go to the “My Friends” section under the Community tab.
  2. Click on the “Add Friends” button. There, you can view suggested friends, find friends by name, and invite friends outside of SJH/COV.
  3. Enter your friends’ names and email addresses on this page. This will generate an automatic email inviting them to join the program. You can even gift your friend a tracking device, if you’d like.

App: Go to the menu and press “Friends” then press on the “+”. The “add friend” option will allow you to add a friend within in SJH/COV. You can search for friends by name or press “+” next to suggested friends.

Monj

What is Monj?

Monj is a brand new online platform that teaches you how to cook healthy meals and have fun in the kitchen! You can earn points by completing activities with Monj.

How can I enroll in Monj?

To get started go the programs page in your Choose Well account. Click on the Monj link.

How can I earn points by participating in Monj?

You can earn points by completing:
  • Monj registration-20 points
  • Daily Missions/quizzes-new one every day--5 points
  • Guided Meals-10 points
  • Essentials-5 points
  • Power Ups-5 points
  • Challenges-200 points (up to 600 points if complete all 3 challenges)

Activity Trackers

What is compatible with Virgin Pulse?

To view compatible activity trackers and apps, go to the “Tracking” tab and click on “Devices & Apps”. The page will show you all devices and APPs that are compatible with Virgin Pulse.

How do I connect my fitness tracker?

Website:

  1. Follow the instructions that come with your activity tracker to set it up.
  2. Log into your Choose Well account go to the Tracking tab and click on “Devices & Apps”.
  3. Click “Connect” next to your activity tracker. This will authorize your activity tracker account to sync to your Virgin Pulse account.

App: Go to the menu and press “Devices & Apps”. Select the device or app you wish to sync to your account and enter the username and password that you used to set up that device or app to authorize it to sync.

Still need help?

  1. Go to the member support page by clicking on the green “Support” tab on the right-hand side of the screen.
  2. Go to the search box. Enter “how to connect my activity tracker”.
  3. If your questions aren’t answered there, you can call member services at (888) 671-9395 or click on “chat” to talk to someone from the support team.

Do I get a discount on trackers?

At the time of enrollment, you will receive a $25 credit to apply toward a device of your choice. All tracking devices in the Virgin Pulse store are discounted.

Where do I go to buy a fitness tracker?

Website: Log into your Choose Well account.

  • Click on “Tracking” in the top-navigation menu. In the drop-down menu that appears, select “Device & Apps”
  • You'll be taken to the "Device & Apps" page, where you'll see all of the options for tracking your activity with a device or mobile application.
  • OR Hover your mouse on your profile picture on the homepage and click “Store”

App: Go to the menu and press “Virgin Pulse Store”.

Events Calendar

What is it?

If you want to know what programs are going on at your ministry, log into your Choose Well account and go to the events calendar

Where can I find it?

  1. Go to the Calender at the top- navigation menu.
  2. Click on Events Calendar

Tax and Payroll

Why am I being taxed on my Choose Well rewards, also known as Virgin Pulse Cash?

We wish we could provide you with tax-free earnings, but are restricted to do so by the Internal Revenue Service (IRS). Virgin Pulse Cash, while issued separately from your St. Joseph Health paycheck, is considered income that is subject to federal and state income tax, based on your withholding status (e.g., single, married, dependent).

Why doesn’t Virgin Pulse take the tax out of my Virgin Pulse Cash?

Virgin Pulse is a vendor contracted by St. Joseph Health and Covenant Health to manage our Choose Well program and they are not your employer, therefore they do not have the ability to deduct tax on your behalf. This means that the Virgin Pulse Cash you accrue and redeem is the full pre-tax amount. The tax on this amount must then be deducted separately by St. Joseph Health on your paycheck.

How often is the tax deducted from my paycheck?

The tax is deducted from your paycheck as you achieve a new level on Virgin Pulse and accrue Virgin Pulse Cash. This tax will show up on your paycheck stub about a month after earnings. You will only be taxed one time after you achieve each level, of which there are four. If you reach several levels in one month, it will all be taxed at once. Once you complete all levels, or you do not move up to a higher level, no additional taxes will be deducted from your paycheck.

How will the tax show up on my paycheck?

The tax amount shown on your paycheck mirrors the Virgin Pulse Cash dollar amount and not the actual tax amount. You can obtain your withholding status by contacting payroll.

I haven’t redeemed my Pulse Cash. Why was I taxed?

Due to IRS regulations, the rewards you accrue in your Virgin Pulse account are still considered income even if you haven’t redeemed them yet. Pulse Cash is yours to redeem whenever you want- as it’s money you have earned. It remains in your Virgin Pulse account even if you leave the organization and haven’t redeemed it.

The tax deducted from my paycheck was a large amount. What happened?

If you earn more than one level in a month, you will see both levels deducted from the same paycheck. Because the tax is deducted once a month, you may have seen a bigger difference in your paycheck.

I’ve been impacted financially because these taxes were deducted.

Choose Well is a financial benefit that is provided to you in addition to your yearly pay because we believe in rewarding you for providing the most compassionate care possible to our patients. Even after deducting taxes, Virgin Pulse Cash is extra income for you on top of your paycheck. We encourage you to redeem your Virgin Pulse Cash.
